How Do I Capture a Client's Signature for Medication Consent or Distribution?

MEDICATION CONSENT/DISTRIBUTION PROCEDURE
Procedure summary:
Medical staff will obtain client’s electronic consent signature for any medication change, new prescription or once per year for unchanged medications.

The procedure for consent and distribution is the same.
If medication change results from a phone call or other situation where the client cannot sign the signature pad complete steps 1-9. After step 9
leave the document unsigned and return to any other section of CareLogic. This will leave the document unsigned, indicating consent is “Pending” on the medication list. Click on the Pending button in the medication list for the medication which needs consent the next time client has an office appointment.

Medical staff will obtain client’s electronic consent signature for any medication change, new prescription or once per year for unchanged medications.
- Medical staff will obtain electronic signature from client for any medication distributed, including Central Pharmacy or sample medication.
- All medications, including samples and Central Pharmacy must be entered in DrFirst for this procedure to be followed.

The procedure for consent and distribution is the same.
- Electronically prescribe medication or enter sample or Central Pharmacy medication in DrFirst.
- Show client printed copy of medication consent.
- From the client’s medication list in CareLogic (in ECR, Show Menu, Medications) click Consent.
- Select medication needing consent from list by clicking box next to medication name.
- Scroll down and click dropdown for Consent Service Document. Select the Medication Consent and Distribution document. Click Submit.
- If client objects or does not consent to medication enter details in box. This step is optional.
- Answer Yes or No to indicate if medication was distributed. Answer is required.
- Guardian Name and Relationship to Client are optional – use if needed, such as distributing medication to patient’s representative.
- Click Submit which takes you to the Signatures tab of the service document.
- Click to Sign, which opens box for signature pad.
- Have client sign pad and click OK.
- Select Guarantor (client) or Enter New Guarantor if needed to indicate who signed signature pad.
- Enter your electronic signature and submit.
