What Are the placement protocols for homeless shelters and crisis units?

PLACEMENT PROTOCOLS

HOMELESS SHELTERS

Rescue Mission

  1. At admission, the provider agency is to begin development of an exit strategy.
  2. The provider agency needs to visit each client at least one time per week.
  3. The provider agency needs to contact the shelter's housing manager at least one time per week to inquire about how the client is doing as well as to discuss the exit strategy.
  4. The provider agency is to coordinate the client's treatment and medication appointments as well as other needs.

CRISIS UNITS 
Riverbend Treatment Center 
Turning Point Crisis Unit

  1. At admission, the provider agency is to begin development of an exit strategy.
  2. The provider agency representative needs to visit in person and/or be in contact by phone with each client at least three (3) times a week. During the visit/phone contact with the client, provider is to also speak/meet with a clinical staff person from the crisis unit to discuss the progress of the client and discuss the exit strategy.
  3. The provider agency is to coordinate the client's treatment and medication appointments as well as other needs.
